You need to include all the copyright notices from any Open Game Content you're using (including Fudge). Then add your own copyright notice customized with the title and copyright information that pertains to you: copyright year and holder -- which would be you or your company if you have a company -- and also list the authors that need to be attributed in any Open Game Content based on your stuff.

That's all you need in files you're uploading to the Fudge Files repository. (The template text includes the Open Game License by link, and adds the stuff you need for compliance with both the OGL and the Fudge System Trademark License.)
